Common Questions

Quick answers.

How do referrals work?

You can reach out yourself using the form above or by phone, or your Service & Support Administrator (SSA) can refer you. Either way, we review the request, call to learn what you need, confirm what services are authorized, and coordinate with your team before support begins. SSAs can find referral details on our For SSAs page.

Who qualifies for services?

Eligibility is set by the funding program, not by us. For people with developmental disabilities, it is generally determined through your county Board of Developmental Disabilities. Older adults are typically assessed through programs such as PASSPORT. Not sure where to start? Call us and we will point you in the right direction.

What does it cost?

Most of our services are paid for through Ohio programs, such as DODD waivers and PASSPORT, rather than billed directly to families. Whether any cost-share applies depends on your program and eligibility, and your SSA or case manager can confirm. If you are unsure what your funding covers, we are happy to help you sort it out.

How fast can care start?

We respond to every request within one business day. When services begin depends on your authorization, your service plan, and matching you with the right staff. Call (419) 245-8113 and we will tell you where things stand and what current availability looks like.

Can I choose my own provider?

Yes. You have the right to choose the provider that is the best fit for you or your family member. We are glad to talk through how we work so you can decide.
